ABSTRACT

Abstract Introduction Otosclerosis is characterized by the fixation of the stapes to the oval window, thereby impairing acoustic signal absorbance. A commonly used surgical technique for improving hearing in cases of otosclerosis is stapedotomy. However, it is unclear whether this surgery restores all the physical characteristics of the tympano-ossicular system. Objective To evaluate the tympano-ossicular system in individuals with fenestral otosclerosis pre and poststapedotomy using wideband tympanometry. Method A total of 47 individuals and 71 ears were assessed. The subjects were divided into three groups: presurgery otosclerosis; postsurgery; and a control group of normal-hearing adults. A handheld tympanometer with a wideband module (226-8,000 Hz) was used to take measurements at ambient pressure and under pressurized conditions. The level of statistical significance adopted was p ≤ 0.05. Results Acoustic absorbance at 226 Hz was low for all groups. At frequencies in the range 630 to 5,040 Hz, each group had a characteristic absorbance curve, allowing them to be distinguished from one another. In the presurgery group, absorbance values were below normal levels, with energy absorbance below 10%. Low energy absorbance was most evident at 1,000 Hz in the presurgery group, but this was not observed in the postsurgery group. Although there was an improvement in hearing, the surgery failed to restore the tympano-ossicular system to normal. Conclusion Wideband acoustic absorbance proved able to differentiate normal ears and otosclerotic ears pre and postsurgery, under both ambient pressure and pressurized conditions.

Abstract Introduction: Endolymphatic hydrops is the pathophysiological substrate of Ménière's disease. The changes in the inner ear, transmitted to the middle ear through changes in the ossicular chain mobility, can be quantified by wideband tympanometry, through the measurement of the acoustic absorbance at multiple frequencies, represented by the sound energy absorbed by the middle ear, even at its early stages. Studying the behavior of the middle ear through the absorbance in patients with endolymphatic hydrops under ambient pressure and under peak pressure can be useful for detecting Ménière's disease. Objective: To characterize acoustic absorbance behavior in subjects with symptomatic and asymptomatic Ménière's disease compared to controls, in order to verify the ability of wideband tympanometry to detect Ménière's disease. Methods: We carried out a cross-sectional study with a diagnostic approach comparing the findings of wideband tympanometry at ambient pressure and peak pressure between the ears of the control group (n = 30), the asymptomatic group (n = 21) and the symptomatic group (n = 9). Results: Different peak pressure values were found between the ears of the control group (0daPa), the asymptomatic group (−11 daPa) and the symptomatic group (−192 daPa), with p < 0.05 by the Kruskal-Wallis test, Mann Whitney test and Bonferroni correction. Different absorbance values were found between the ears of the symptomatic group and the asymptomatic group compared to the control group for low frequencies at ambient pressure and peak pressure, with p < 0.05 by the Kruskal-Wallis test, Mann Whitney test and Bonferroni correction. Conclusions: The Wideband Tympanometry test was capable of identifying the presence of Ménières disease, and to differentiate between asymptomatic and symptomatic patients, when comparing them with healthy individuals.


Resumo Introdução: A hidropsia endolinfática é o substrato fisiopatológico da doença de Ménière. As alterações desencadeadas na orelha interna, transmitidas à orelha média pelas modificações na mobilidade da cadeia ossicular, podem ser quantificadas pela timpanometria de banda larga, através da medida da absorvância acústica sob múltiplas frequências, representadas pela energia sonora absorvida pela orelha média, mesmo em estágios iniciais de sua instalação. Estudar o comportamento da orelha média através da absorvância em pacientes com hidropisia endolinfática sob pressão ambiente e sob o pico de pressão pode ser útil na detecção da doença de Ménière. Objetivo: Caracterizar o comportamento da absorvância em indivíduos com diagnóstico da doença de Ménière sintomáticos e assintomáticos, comparados com controles, a fim de verificar a capacidade da timpanometria de banda larga em detectar variações clínicas relacionadas a possível hidropisia endolinfática. Método: Foi realizado um estudo transversal, com enfoque diagnóstico, que comparou os achados da timpanometria de banda larga na pressão ambiente e no pico de pressão entre orelhas do grupo controle (n = 30), grupo assintomático (n = 21) e grupo sintomático (n = 9). Resultados: Foram encontrados valores do pico de pressão diferentes entre orelhas do grupo controle (0 daPa), do grupo assintomático (-11 daPa) e do grupo sintomático (-192 daPa), com p <0,05 pelos testes de Kruskal-Wallis, Mann Whitney e correção de Bonferroni. Foram encontrados valores de absorvância diferentes entre orelhas do grupo sintomático e do grupo assintomático em relação ao grupo controle para as frequências baixas na pressão ambiente e na pressão de pico, com p < 0,05 pelos testes de Kruskal-Wallis, Mann Whitney e correção de Bonferroni. Conclusão: A timpanometria de banda larga foi um teste capaz de identificar a presença da doença de Ménière e de diferenciar os pacientes assintomáticos e sintomáticos, comparando-os com indivíduos hígidos.

RESUMEN Introducción y objetivos : Determinar si el tipo de fisura labio palatina está asociada a mayor riesgo de alteración de la trompa de Eustaquio. Materiales y métodos : Se realizó un estudio descriptivo serie de casos con análisis inferencial de niños operados de fisura labiopalatina en el Hospital San Bartolomé durante el 2018 y 2019. Resultados : Se evaluó a un total de 48 pacientes a quienes se les realizó timpanometría, encontrándose 62,5 % varones (n=30) y 37,5 % mujeres (n=18) y la media de edad fue de 1,3 años, para determinar el tipo de fisura labiopalatina se usó la clasificación de Veau, el tipo más frecuente hallado fue Veau tipo III, con 62,5 % (n=30), siendo la menos frecuente Veau tipo I con 4,2 % (n=2). La evaluación de la función de la trompa de Eustaquio se realizó mediante timpanometría definiéndose como patológica la curva tipo B, en el grupo de estudio se encontró una prevalencia global de curvas tipo B de 66,7 % (n= 32), mientras que las curvas A y C, tuvieron 33,3 % (n=16). La probabilidad que el diagnóstico Veau IV tenga curvas tipo B de forma más frecuentes que los otros tipos, mediante la prueba Binomial fue significativo. (p=0,000038<0,05). Conclusiones : El tipo de fisura palatina mayormente asociado con alteración de la función del oído medio, expresado mediante curvas de timpanometría tipo B, es la fisura tipo IV según clasificación de Veau, esta relación es estadísticamente significativa.


ABSTRACT Introduction and objectives : The objective of this study is to determine if the type of cleft lip and palate is associated with a higher risk of alteration of the Eustachian tube. Materials and methods : A descriptive case series study was carried out with an inferential analysis of children operated on for cleft lip and palate at the San Bartolomé hospital during the years 2018 and 2019. Results : 48 patients with tympanometry were evaluated, 62.5% male (n = 30) and 37.5% female (n = 18), the mean age was 1.3 years. The type of cleft lip and palate was determined using the Veau classification, the most frequent type found was Veau type III: 62.5% (n = 30), the least frequent type was Veau I with 4.2% (n = 2). The evaluation of the function of the Eustachian tube was performed with tympanometry, defining the type B curve as pathological. In the study group, there was an overall prevalence of type B curves of 66.7% (n = 32), while type A and C curves were 33.3% (n = 16). The probability that the Veau IV crack type has an association with type B curves more frequently than the other types, using the Binomial test, was significant. (p = 0.000038 <0.05). Conclusions : The type of cleft palate that is most associated with impaired middle ear function, expressed by type B tympanometry curves, is type IV cleft according to Veau's classification, this relationship is statistically significant.

RESUMO Objetivo Analisar as emissões otoacústicas evocadas produto de distorção (EOAPD) de indivíduos adultos entre 18 e 50 anos, com audição normal, e associar os resultados com os achados à timpanometria. Métodos Foram selecionados 27 prontuários de adultos com audição dentro dos padrões de normalidade, sem queixa auditiva, com curva timpanométrica do tipo A, Ad ou Ar, presença de reflexos acústicos, sem queixa de zumbido e de exposição frequente a níveis de pressão sonora elevados, com repouso auditivo mínimo de 14 horas no momento do exame e que realizaram o exame de emissões otoacústicas evocadas produto de distorção. Os resultados do exame de emissões otoacústicas foram analisados considerando os resultados das curvas timpanométricas apresentadas por estes indivíduos. Para análise dos dados foram aplicados testes não paramétricos e o nível de significância foi de 5%. Resultados foram analisados os resultados das emissões otoacústicas de 54 orelhas. Observou-se maior ocorrência da curva do tipo A em indivíduos sem queixas auditivas. Independente do lado, a maioria das orelhas que apresentou resposta presente ao exame de EOAPD, apresentou, também, curva timpanométrica normal. Observou-se correlação positiva entre a amplitude das EOAPD e o volume da orelha média para as frequências de 6000 Hz e uma tendência à significância em 4000 Hz Conclusão Foi possível concluir que há maior ocorrência de emissões otoacústicas presentes em indivíduos com audição normal e curva timpanométrica do tipo A e que a amplitude das EOAPD em 6000 Hz mostra-se menor nos indivíduos com audição normal e curva timpanométrica do tipo Ar ou Ad.


ABSTRACT Purpose To analyze distortion product evoked otoacoustic emissions in normal-hearing adults aged between 18 and 50 years old, and to associate the results with the findings of tympanometry. Methods 27 medical records were selected of adults with the following conditions: normal hearing; without auditory complaint; with type A, Ad or Ar tympanometric curve; with presence of acoustic reflexes; with no complaint of tinnitus or frequent exposure to high sound pressure levels; with minimal auditory rest of 14 hours at the time of the test, and who had undergone distortion product evoked otoacoustic emission (DPOAE) testing. The results of otoacoustic emissions were analyzed considering the results of the tympanometric curves presented by these individuals. For the data analysis, non-parametric tests were applied, and the level of significance was 5%. Results The results of otoacoustic emissions of 54 ears were analyzed. There was a greater occurrence of the type A curve in individuals without auditory complaints. Regardless of side, most ears whose response was present in the DPOAE test also presented normal tympanometric curve. There was a positive correlation between DPOAE amplitude and middle ear volume for the 6000Hz frequencies (p = 0.048) and a tendency to significance at 4000Hz (p = 0.054). Conclusion There was a higher occurrence of otoacoustic emissions present in normal-hearing individuals and type A tympanometric curve, and the amplitude of DPOAE at 6000Hz was smaller in normal-hearing individuals and type Ar or Ad tympanometric curve.

Abstract Introduction: Tympanometry is currently the most frequently used tool for assessing the status of the middle ear, commonly assessed using a single 226 Hz tone. However, the use of the Acoustic Immittance Measures with a wideband stimulus is a promising high-resolution evaluation, especially in individuals known to have middle ear alterations, such as Down syndrome patients. Objective: The aim of this study was to analyze the acoustic absorbance measurements in children with Down syndrome. Methods: Cross-sectional study, approved by the institution's ethics committee. Data were collected from 30 children, with a mean age of 8.4 years, 15 with Down syndrome (DS-study group) and 15 children with typical development and no hearing complaints (control group). Energy absorbance was measured at frequencies of 226-8000 Hz at ambient pressure and at peak pressure as a function of frequency using TITAN equipment. Statistical analysis was performed using the established level of statistical significance of 5%. Results: With the 226 Hz probe tone, 30 ears of the control group and 22 of the study group exhibited Type A tympanograms, whereas Type B was observed in eight children in the study group. The mean acoustic absorbance ratio of the study group was lower than that of the control group at frequencies centered at 2520 Hz (p = 0.008) for those with normal tympanometry results, and 226-4000 Hz (p < 0.03) for those with a Type B tympanometry curve. Conclusion: The low energy absorption in the presence of normal tympanograms in children with Down syndrome may suggest middle ear abnormalities.


Resumo Introdução: A timpanometria é a ferramenta mais usada para avaliar o status da orelha média, comumente avaliada por meio de uma única frequência com o tom de 226 Hz. No entanto, o uso da medida de imitância acústica com estímulo de banda larga é uma avaliação de alta resolução promissora, especialmente em pacientes conhecidos por frequentemente apresentar alterações da orelha média, como na síndrome de Down. Objetivo: Analisar as medidas de absorvância acústica em crianças com síndrome de Down. Método: Estudo transversal, aprovado pelo comitê de ética da instituição. Foram coletados dados de 30 crianças, com idade média de 8,4 anos, sendo 15 com síndrome de Down (SD-grupo de estudo) e 15 crianças desenvolvimento típico e sem queixas auditivas (grupo controle). A absorvância de energia foi medida nas frequências de 226-8.000 Hz à pressão ambiente e no pico de pressão em função da frequência, usou-se o equipamento Titan. A análise estatística foi feita com o nível de significância estatística adotado de 5%. Resultados: Com o tom de sonda de 226 Hz foram observadas 30 orelhas do grupo controle e 22 do grupo estudo com timpanometria Tipo A e o Tipo B foi observado apenas em oito crianças do grupo estudo. A razão média de absorvância acústica do grupo estudo foi menor do que a do controle nas frequências centradas em 2.520 Hz (p = 0,008) para aqueles com resultados timpanométricos normais e de 226-4.000 Hz (p < 0,03) para aqueles com curva timpanométrica Tipo B. Conclusão: A baixa absorção de energia na presença de timpanogramas normais nas crianças com síndrome de Down pode sugerir anormalidades na orelha média.

ABSTRACT

OBJECTIVES: We explored whether wideband tympanometry (WBT) could be used as a screening test for superior semicircular canal dehiscence (SSCD), and obtained new WBT data (given that the test is not yet in common clinical use) on patients with SSCD. METHODS: We compared the WBT data of patients clinically and radiologically diagnosed with SSCD in our hospital between 2013 and 2018 to those of healthy volunteers. We compared the resonance frequency (RF), maximum absorbance frequency (MAF), and maximum absorbance ratio (MAR). The t-test was used for statistical analysis with the significance level set to P<0.05. In addition, we used receiver operating characteristic analysis to derive cutoff values for SSCD diagnosis in terms of sensitivity and specificity. RESULTS: Seventeen patients (four with bilateral and 13 with unilateral disease; 17 ears) diagnosed with SSCD and 27 healthy volunteers (47 ears) were included. The mean RFs of the SSCD patients and healthy subjects were 548.7 Hz (range, 243 to 853 Hz) and 935.1 Hz (range, 239 to 1,875 Hz), respectively (P<0.001). The mean MARs of the SSCD patients and healthy subjects were 89.4% (range, 62% to 100%) and 82.4% (range, 63% to 99%), respectively (P=0.005). The mean MAFs of the SSCD patients and healthy subjects were 1,706.3 Hz (range, 613 to 3,816 Hz) and 2,668 Hz (range, 876 to 4,387 Hz), respectively (P<0.001). In terms of SSCD diagnosis, a MAR above 86% afforded 81% sensitivity and 77% specificity; an RF below 728 Hz, 86% sensitivity and 81% specificity; and an MAF below 1,835 Hz, 79% sensitivity and 67% specificity. CONCLUSION: WBT may be a useful clinical screening test for SSCD. The RF and MAF were lower, and the MAR higher, in SSCD patients than in normal controls.

ABSTRACT

OBJECTIVE To analyse the result of mastoid abnormal MRI singals in infants without clinical symptoms and to evaluate the diagnostic value. METHODS The MRI data of abnormal signals in the middle ear and mastoid of 42 infants(62 ears) were analyzed with 1000 Hz probe tone tympanometry and oto-endoscope. RESULTS Of the infants with abnormal MRI signals of the middle ear and mastoid, 50 ears were secretory otitis media(80.7%), 10 ears had dysfunction of middle ear (16.1%), and 2 ears were not identified. CONCLUSION In the infant who had a abnormal long T2 singals MRI but without clinical symptoms, 80.7% were caused by SOM or AOM. It is valuable for clinical efficacy evaluation and treatment planning in advance.

Evaluation of middle ear function in infants is the key to distinguish sensorineural hearing loss and conductive hearing loss, and acoustic immittance test is the routine audiological evaluation of middle ear function.Because of the characteristics of middle ear in infants, middle ear examination parameters of adults are not suitable for infants. This article reviewd the current multiple acoustic immittance methods for detecting middle ear function in infants, and summarized the advantages and disadvantages of these methods.

Objective To obtain suitable hearing screening methods for preschool children.Methods A total of 2025 children aged 2~6 years old in 30 kindergartens in Huangshi City were selected by random sampling method.Acoustic impedance and otoacoustic emission tests(transiently evoked otoacoustic emissions and distortion product otoacoustic emissions) were performed in two stages of preliminary hearing screening.The children who failed the hearing screening needed to do the re-creening with the same methods;the children who failed the rescreening needed to receive audiological tests including ABR, ASSR examination and imaging examinations.Results The total screening pass rate was 94.02%, of which 1 842 passed the preliminary hearing screening(90.96%, 1 842/2 025).The 183 children who failed the preliminary hearing screening received the re-screening, 62 children passed the re-screening(33.88%,62/183).121 children failed the re-screening(5.98%,121/2 025), and finally 72 children(3.56%,72/2 025)were diagnosed with hearing loss, including 47 cases of otitis media,22 cases of sensorineural hearing loss(8 cases were moderate, 4 cases were severe hearing loss,10 cases were profound);18 cases were unilateral hearing loss while 4 cases were bilateral hearing loss.Conclusion Acoustic impedance and otoacoustic emission tests can be used for hearing screening in preschool children.The hearing problems of preschool children in Huangshi City were concentrated mainly in the middle ear secretory otitis media and different degree of sensorineural hearing loss.

Na prática clínica, nota-se uma dificuldade em identificar a rigidez do sistema tímpano-ossicular decorrente da Otosclerose por meio dos métodos de avaliação rotineiramente empregados. Assim, as medidas de imitância acústica de banda larga podem fornecer mais informações sobre a condição do sistema tímpano-ossicular por avaliarem uma ampla faixa de frequência. O objetivo do estudo foi caracterizar as medidas de imitância acústica de banda larga com os estímulos tom puro e chirp em indivíduos com diagnóstico de Otosclerose submetidos à estapedectomia ou estapedotomia. Foram avaliados 40 indivíduos na faixa etária de 33 a 80 anos, totalizando 54 orelhas operadas e 22 orelhas não operadas. A avaliação audiológica foi realizada por meio da otoscopia, audiometria tonal liminar, logoaudiometria e imitanciometria. As medidas de imitância acústica de banda larga foram obtidas por meio do sistema de medidas Middle-Ear Power Analyzer MEPA3, versão 5.0 (Mimosa Acoustics), utilizando os estímulos tom puro e chirp. Os dados obtidos foram submetidos à análise estatística descritiva e inferencial, com nível de significância de 0,05. As orelhas com Otosclerose apresentaram maior absorvância em 750 Hz, uma tênue diminuição até 3000 Hz, acentuando-se a partir desta frequência. A magnitude da admitância foi maior na faixa de frequência de 1992 a 4008 Hz, assim como a magnitude da impedância foi maior nas frequências baixas com diminuição até 4008 Hz e aumento em 6000 Hz. Estas orelhas foram dominadas pela rigidez nos sons graves e médios, por resistência entre 3000 e 4008 Hz e a partir desta frequência pela massa. A fase da impedância foi maior em 258 Hz e o delay da reflectância (slope) apresentou uma variação de comportamento entre as frequências, não sendo possível caracterizar um padrão de resposta. Conclui-se que há diferenças nas medidas obtidas nas orelhas com Otosclerose submetidas à cirurgia, quando analisadas as frequências baixas e médias, o que demonstra o impacto positivo da intervenção cirúrgica na funcionalidade do sistema tímpano-ossicular.(AU)


In clinical practice, there is a difficulty in identifying the rigidity of the tympano-ossicular system resulting from Otosclerosis through the routinely employed evaluation methods. Thus, wideband acoustic immitance measurements can provide more information on the condition of the tympanic-ossicle system by evaluating a wide frequency range. This study aimed at characterizing the wideband acoustic immitance measurements with the pure tone and chirp stimuli in individuals diagnosed with Otosclerosis submitted to stapedectomy or stapedotomy. Forty individuals aged 33 to 80 years were evaluated, totaling 54 operated and 22 non-operated ears. Audiology assessment was performed through otoscopy, pure-tone audiometry, logoaudiometry and imitanciometry. Wideband acoustic immitance measurements were obtained using the Middle-Ear Power Analyzer (MEPA3), version 5.0 (Mimosa Acoustics), by means of pure tone and chirp stimuli. Data were submitted to descriptive and inferential statistical analysis, with a significance level of 0.05. The ears with Otosclerosis presented a greater absorbance in 750 Hz, a slight diminution until 3000 Hz, being accentuated from this frequency. The admittance magnitude was higher in the frequency range from 1992 to 4008 Hz, as well as the impedance magnitude was higher in the low frequencies with a decrease up to 4008 Hz and an increase in 6000 Hz. These ears were dominated by rigidity in the low and medium sounds, by resistance between 3000 and 4008 Hz, and from this frequency, by the mass. The impedance phase was higher at 258 Hz and the reflectance delay (slope) showed a behavioral variation between the frequencies, a response not being characterized. It can be concluded that there are differences in the measurements obtained in Otosclerosis ears submitted to surgery, when the low and medium frequencies were analyzed, which demonstrates the positive impact of the surgical intervention on the tympano-ossicle system.(AU)

OBJECTIVES: The aim of this study was to establish the rate variation of sonotubometric measurements using a specific broadband class of signals, the so-called perfect sequences (PSEQ) among healthy adults and to identify an optimal and technically simple test to provoke Eustachian tube (ET) openings. METHODS: Sonotubometry was performed on 105 healthy adult subjects. Three different consecutive maneuvers were performed for ET opening: dry swallowing, water swallowing (a small [2 mL] and a large [5 mL] water bolus). Values of the amplitude and duration of each measured ET opening were calculated. RESULTS: A total of 6,300 measurements were performed. Sonotubometric ET openings were detected for all subjects but not for each measurement. In 6,180 of 6,300 measurements (98.1%), objective ET openings were registered. In 11 of 105 subjects (10.5%) at least one sonotubometric ET opening was not detected. The mean ET opening duration time and the mean sound amplitude similar for all performed test and were 270 (SD, ±96) msec, 13.48 (SD, ±6.57) dB. CONCLUSION: Sonotubometry based on PSEQ stimuli is a reliable methodology to assess the ET opening function in healthy subjects. Mean ET opening duration time and the mean sound wave amplitude performed similarly in all analysed tests, hence it might be concluded that dry (saliva) and water swallowing are reliable sonotubometric maneuvers and may be used when examining ET opening function. The size of a sip during water swallowing does not affect the sonotubometry result. All maneuvers can be equally used as the optimal test, and water swallow is most comfortable for the subject.

RESUMO Objetivo: Caracterizar os valores de reflectância e absorvância de banda larga em lactentes com integridade de orelha média. Métodos: Foram analisados os valores de reflectância e absorvância de banda larga, para os estímulos chirp e tom puro, de 31 orelhas de 18 lactentes, na faixa etária de 10 dias a cinco meses de idade. Considerou-se como critérios de inclusão: ausência de fator de risco para deficiência auditiva, timpanometria com sonda de 1000 Hz, sugerindo normalidade de orelha média e presença de emissões otoacústicas evocadas por estímulo transiente, na triagem auditiva neonatal. Resultados: Os valores de reflectância mostraram-se maiores para as baixas frequências, reduzindo para as médias e aumentando, novamente, para as frequências altas, enquanto para os valores de absorvância, o comportamento foi o oposto. Não existiu diferença significativa entre os estímulos chirp e tom puro. Conclusão: Observou-se um comportamento típico das medidas estudadas, caracterizado por maior reflectância nas frequências graves e maior absorvância para as frequências médias.


ABSTRACT Purpose: To characterize the wideband reflectance and absorbance values in infants with middle ear integrity. Methods: The wideband reflectance and absorbance values for chirp and pure tone stimuli of 31 ears of infants aged 10 days to five months were analyzed. Inclusion criteria considered: 1000 Hz tympanometry. suggesting normal middle ear and the presence of transient evoked otoacoustic emission. Results: The reflectance values were shown to be greater for low frequencies, reducing to medium ones and increasing once again to high frequencies, while the absorbance values displayed an opposite behavior. No significant difference was verified between chirp and pure tone stimuli. Conclusion: A typical behavior of the measures, characterized by a higher reflectance at low frequencies and higher absorbance at medium frequencies was observed in the population studied.

Resumo:OBJETIVO:caracterizar o perfil audiológico de escolares do primeiro ano do Ensino Fundamental da rede municipal da cidade de Caxias do Sul-RS, investigando e identificando a ocorrência de alterações auditivas nesta população, por meio de medidas eletroacústicas.MÉTODOS:estudo transversal, prospectivo, descritivo e quantitativo de 391 escolares do primeiro ano do Ensino Fundamental de escolas municipais da cidade de Caxias do Sul-RS. Os estudantes foram submetidos a quatro procedimentos audiológicos na Triagem Auditiva Escolar: Meatoscopia, Emissões Otoacústicas Evocadas por Estímulo Transiente, Emissões Otoacústicas - Produto de Distorção e Timpanometria. Além disso, os responsáveis pelas crianças responderam a um questionário com perguntas referentes ao desenvolvimento e aos antecedentes otológicos de cada criança. Os escolares que apresentaram alguma alteração auditiva na Triagem Auditiva foram encaminhados para avaliação otorrinolaringológica e/ou avaliação audiológica completa.RESULTADOS:das 782 orelhas avaliadas, 85,4% apresentaram normalidade nos exames realizados e 14,6% demonstraram algum tipo de alteração. Na Meatoscopia, verificou-se que 6,3% das orelhas apresentaram alterações. Passaram nas Emissões Otoacústicas Evocadas por Estímulo Transiente 94,8% das orelhas e nas Emissões Otoacústicas - Produto de Distorção 95,8%. Na Timpanometria, a curva predominante foi a tipo A, seguida pela tipo C. Foram realizados encaminhamentos para 14,6% dos escolares.CONCLUSÃO:a população desse estudo foi composta, em sua maioria, por escolares do gênero masculino, com seis anos de idade. Das 782 orelhas avaliadas, 85,4% apresentaram normalidade nos exames realizados e 14,6% demonstraram algum tipo de alteração.


Abstract:PURPOSE:to characterize the audiological profile of first grade students from municipal elementary schools in Caxias do Sul (RS), Brazil, using electroacoustic measures to investigate and identify the occurrence of hearing impairments in this population.METHODS:prospective descriptive quantitative cross-sectional study of 391 first grade students from municipal elementary schools in Caxias do Sul (RS), Brazil. Students were submitted to four audiological procedures for the hearing screening: meatoscopy, Transient Evoked Otoacustic Emissions, Distortion Product Otoacoustic Emissions, and tympanometry. Furthermore, caregivers answered a questionnaire regarding each child's development and otologic history. The students who presented hearing deficits on the hearing screening were referred for otorhinolaryngological and/or complete audiological evaluation.RESULTS:from the 782 ears evaluated, 85.4% presented normal results, and 14.6% showed alterations. In the meatoscopy, 6.3% of the ears showed alterations, while 94.8% passed the Transient Evoked Otoacustic Emissions and 95.8%, the Product-Distortion Otoacoustic Emissions. The tympanometry showed predominance of type A curves, followed by type C. Referrals were made for 14.6% of the students.CONCLUSION:the sample of this study was composed mostly by male school-age children with six years of age. From the 782 ears evaluated, 85.4% had normal results on the screening, and 14.6% showed alterations.

PURPOSE: To analyze the occurrence of acoustic reflex and its threshold on newborns using the 226 and 1,000 Hz probes. METHODS: Thirty-six newborns with "PASS" results in newborn hearing screening and tympanogram with one or two peaks for both probe tones were included. Group I comprised 20 full-term newborns without risk indicator for hearing loss, and Group II comprised 16 newborns with at least one risk indicator. The study about ipsilateral acoustic reflex thresholds was conducted in 500, 1,000, 2,000, and 4,000 Hz. RESULTS: The groups presented the acoustic reflex thresholds between 50 and 100 dB for both probe tones. In the comparison between the probes, there were differences in all frequencies evaluated in Group I, with the lowest threshold mean for the 1,000 Hz probe. In Group II, differences were detected at 2,000 Hz. The mean acoustic reflex thresholds were similar in both groups for the 226 Hz probe. There was a difference for the 1,000 Hz probe in all tested frequencies. The percentage of response was higher in both groups for the 1,000 Hz probe. The kappa test showed extremely poor agreement in the comparison of results between both probes. CONCLUSION: The occurrence of acoustic reflex was higher in newborns and its thresholds were lower with the 1,000 Hz probe both for healthy newborns and for newborns at risk. .


OBJETIVO: Verificar a ocorrência e o limiar do reflexo acústico em neonatos utilizando a sonda de 226 e 1.000 Hz. MÉTODOS: Trinta e seis neonatos com resultado "PASS" na triagem auditiva neonatal e timpanogramas com um ou dois picos para os dois tons de sonda. O Grupo I foi composto por 20 neonatos a termo sem indicador de risco para deficiência auditiva e o Grupo II, por 16 neonatos com pelo menos um indicador de risco. A pesquisa dos limiares do reflexo acústico ipsilateral foi realizada em 500, 1.000, 2.000 e 4.000 Hz. RESULTADOS: Os grupos apresentaram limiar do reflexo acústico entre 50 e 100 dB para ambos os tons de sonda. Na comparação entre as sondas, houve diferença em todas as frequências avaliadas no Grupo I, com a média de limiares menor com a sonda de 1.000 Hz. No Grupo II, foi detectada diferença em 2.000 Hz. Com a sonda de 226 Hz, os limiares médios do reflexo acústico foram semelhantes nos dois grupos. Com a sonda de 1.000 Hz, houve diferença em todas as frequências avaliadas. A porcentagem de presença de resposta foi maior para ambos os grupos para a sonda de 1.000 Hz. O teste kappa revelou concordância extremamente pobre na comparação dos resultados entre as duas sondas. CONCLUSÃO: A ocorrência de reflexo acústico em neonatos foi maior e os limiares menores com a utilização da sonda de 1.000 Hz, tanto para neonatos saudáveis como para os neonatos de risco. .

INTRODUCTION: Controversies arise with respect to functioning of the middle ear over time. OBJECTIVE: To assess changes in middle ear impedance that may be related to aging, and/or if there was an association of these changes with those of the inner ear in the elderly patients. METHODS: Cross-sectional, comparative study of elderly patients managed in ear, nose and throat clinics. A structured questionnaire was administered to obtain clinical information. Pure tone audiometry, tympanometry, and acoustic reflexes were performed. Comparative analyses were performed to detect intergroup differences between clinico-audiometric findings and middle ear measures, viz. tympanograms and acoustic reflexes. RESULTS: One hundred and three elderly patients participated in the study; 52.4% were male, averagely 70.0 ± 6.3 years old, age-related hearing loss in 59.2%, abnormal tympanograms in 39.3%, absent acoustic reflex in 37.9%. There was no association between age and gender in patients with abnormal tympanograms and absent acoustic reflex. Significantly more patients with different forms and grades of age-related hearing loss had abnormal tympanometry and absent acoustic reflex. CONCLUSION: Some abnormalities were observed in the impedance audiometric measures of elderly patients, which were significantly associated with parameters connected to age-related hearing loss. .


INTRODUÇÃO: Existem controvérsias no que se refere às alterações funcionais da orelha média com o passar dos anos. OBJETIVO: Avaliar as mudanças na impedância da orelha média que podem estar relacionadas ao envelhecimento, bem como qualquer associação dessas alterações com as que ocorrem na orelha interna. MÉTODO: Estudo prospectivo comparativo de pacientes idosos atendidos em ambulatórios espe-cializados em otorrinolaringologia e aplicação de questionário estruturado para obtenção de informações clínicas. Foram realizadas audiometria de tons puros, timpanometria e reflexos acústicos e análise comparativa para detectar as diferenças intergrupos entre os achados clínico-audiométricos. RESULTADOS: Participaram do estudo 103 pacientes idosos: 52,4% do gênero masculino; idade de70 ± 63 anos; perda auditiva relacionada à idade detectada em 59,2%; timpanograma anormal em 39,3%; e reflexo acústico ausente em 37,9%. Não foi encontrada associação entre idade e gênero em pacientes com timpanograma anormal e reflexo acústico ausente. Um número significantemente maior de pacientes com diferentes graus e configurações de perda auditiva relacionada à idade apresentou timpanometria anormal e reflexo acústico ausente. CONCLUSÃO: Algumas anormalidades foram observadas em medidas de impedância audiométrica em pacientes idosos, que foram significantemente associados com os parâmetros ligados à perda auditiva relacionada à idade. .

A avaliação da integridade do sistema tímpano-ossicular é um pré-requisito para que o processo de diagnóstico audiológico infantil possa ser concluído com precisão, assim, muito têm se discutido em relação aos procedimentos utilizados na prática clínica para definir a presença ou ausência de problema condutivo. Nesse sentido, as medidas de imitância acústica de banda larga mostram-se como uma opção para o avanço na área. Diante disso, surgiu o interesse em ampliar os conhecimentos e trazer as características das medidas de imitância acústica de banda larga nas diferentes condições de orelha média. O objetivo do estudo foi caracterizar as medidas de imitância acústica de banda larga com os estímulos tom puro e chirp em crianças com perda auditiva sensorial de grau severo e profundo e diferentes padrões timpanométricos. Foram avaliadas 187 orelhas de 98 crianças na faixa etária de seis a 60 meses de idade, subdivididas em grupos de acordo com a pressão e compliância obtidas na timpanometria com sonda de 226 Hz. As medidas de imitância acústica de banda larga foram realizadas por meio do sistema de medidas do Acoustics' Middle-Ear Power Analyzer - MEPA, versão 5.0, utilizando os estímulos tom puro e chirp, com faixa de frequência de 211 a 6000 Hz. Os valores de compliância e pressão obtidos na timpanometria convencional foram correlacionados às medidas de imitância acústica de banda larga, adotando-se testes estatísticos apropriados, com nível de significância de 0,05. Os resultados obtidos evidenciaram o efeito das faixas de frequência (grave, média e aguda) em todas as medidas analisadas, com exceção do delay da reflectância. Foi constatada influência de pressões correspondentes a -100 daPa ou mais negativas e também para pressões positivas acima de 25 daPa, e compliâncias menores que 0,2 ml nas medidas de imitância acústica de banda larga. Além disso, houve diferença significante entre os estímulos tom puro e chirp para frequências baixas...

OBJECTIVES: The aim of this study was to investigate hearing and outer cells function in patients with psoriatic arthritis. Our investigation was a prospective case control study. METHODS: A total of 31 psoriatic arthritis patients (62 ears) and 31 healthy control subjects (62 ears) were enrolled in the study. We investigated hearing changes of patients and controls via pure tone audiometry, speech discrimination scores, tympanometry, acoustic reflex, and transient product otoacoustic emission. RESULTS: The mean age of psoriatic arthritis patients was 36.1+/-8.5 years (range, 14 to 62 years). The average age of the control group was 37.9+/-8.1 years (range, 16 to 62 years). There were statistically significant differences between pure tone audiometry in all frequencies and right and left emission at the 4.0 and 1.0 in psoriatic arthritis patients versus controls (P0.05). Both audiological and otoacoustic emissions were not significantly different between right and left ear (P>0.05). CONCLUSION: Based on the audiological and otoacoustic findings; it is likely that the cochlear outer hair cells become subtly damaged in psoriatic arthritis patients, consequently leading to changes in hearing thresholds. These data suggest that it is important to screen psoriatic arthritis patients for hearing changes with otoacoustic emissions and audiologic tests regularly.

ABSTRACT

OBJETIVO: verificar a ocorrência de simetria maior ou igual à 70%, entre as orelhas, comparando os resultados da timpanometria nas frequências de 226 e 1000Hz com as respostas das emissões otoacústicas em neonatos. MÉTODOS: foram avaliados 39 neonatos, em média com 60 horas de vida, sendo 20 do sexo feminino e 19 do sexo masculino. Cada recém-nascido foi submetido à avaliação timpanométrica com as sondas de 226 e 1000 Hz, e avaliação das emissões otoacústicas transientes em ambas as orelhas. Os resultados foram submetidos a testes estatísticos. RESULTADOS: na análise da amostra pode-se observar ocorrência de simetria <70% nas respostas das emissões otoacústicas em 74,4% do total de neonatos. Por outro lado, na timpanometria, houve uma maior ocorrência de simetria ≥ 70%, tanto para sonda de 226 quanto para 1000Hz (76,9% e 84,6%, respectivamente). No que diz respeito ao gênero e orelha, as diferenças encontradas em cada teste não foram significantes, embora tenha sido observada maior amplitude de respostas de emissões otoacústicas na orelha direita. CONCLUSÃO: os resultados sugerem haver simetria no sistema auditivo em sua porção mais periférica, e início de assimetrias a partir da cóclea. .


PURPOSE: to determine the occurrence of symmetry ≥ 70% between the ears, comparing the results of 226- and 1000-Hz tympanograms with otoacoustic emissions in neonates. METHODS: thirty-nine neonates (20 female and 19 male) were evaluated at an average age of 60 hours. Each newborn underwent 226- and 1000-Hz probe tone tympanometry and evaluation of the transient-evoked otoacoustic emissions in both ears. The results were subjected to statistical tests. RESULTS: occurrence of symmetry < 70% was observed in otoacoustic emissions for 74.4% of the neonates. Tympanometry produced a greater occurrence of symmetry ≥ 70% for both the 226- and 1000-Hz probe tones (76.9% and 84.6%, respectively). With regard to gender and ear, the differences in each test were not significant, although greater otoacoustic emission amplitude was observed for the right ear. CONCLUSION: the results suggest symmetry in the auditory system in its most peripheral portion, and asymmetries begin to occur from the cochlea. .

A paciente, do gênero feminino, 51 anos, apresentava queixa de hipoacusia bilateral e a hipótese diagnóstica de otosclerose. Foram realizados os seguintes testes de avaliação audiológica: imitanciometria, audiometria tonal, vocal e medidas de reflectância acústica, em ambas as orelhas. Por meio da análise dos resultados da reflectância acústica de banda larga, foi possível observar aumento na reflectância, característico de otosclerose, entre as frequências de 500 e 1500 Hz, e diferenciar a orelha esquerda e a orelha direita. As medidas de reflectância permitiram a obtenção de resultados detalhados por frequência que, em conjunto, possibilitaram a diferenciação entre as duas orelhas. O uso da reflectância de banda larga é sugerido para complementar o diagnóstico da otosclerose.


This research aimed to analyze the reflectance measurements in a patient with otosclerosis. The patient, female, 51 years old; she complained of bilateral hearing loss and had a diagnosis hypothesis of otosclerosis. The following audiological tests were performed in both ears: tympanometry, pure-tone audiometry, and speech audiometry tests and acoustic reflectance measurements. Through acoustic wideband reflectance analysis, it was possible to observe an increase in reflectance, such as is seen in otosclerotic ears, between the frequencies of 500 Hz and 1500 Hz and to differentiate the left and right ear. Acoustic reflectance measurements have the potential to yield results that allow the differentiation between the two ears; thus, the use of the wideband acoustic reflectance is suggested as a part of the otosclerosis diagnostic procedure.
